Travel Exceptions

Do I Have to Fill out the Authorization to Travel Form?

Most travelers are required to fill out this form for every business trip. (*there is one exception - see below). This form consolidated all 'prior written approval' requests previously done by memo. During the planning stages of your trip, please enter your estimated expenses on the Authorization to Travel form. Also, use the form to make any special travel requests such as international travel or car rental.

If you check off any of the special request boxes on the authorization form, you must attach the Authorization Form to the Travel Voucher when you submit it:

The special requests are:

  • Car rental request.
  • Navigation equipment request. Traveler must justify request.
  • Routine lodging overage request. Traveler must justify request.
  • Actual conference hotel rate. Traveler must justify request
  • Inclusion of suburbs.
  • Single day meal reimbursement request.
  • Actual meal cost reimbursement request.
  • International travel request.
  • Temporary assignment extension request.

    If you don't check off any of the special request boxes on the authorization form, all you have to do is get your budgetary head (the person whose money is being spent) to sign the form and file it in your department.

    *However, there is one exception to this rule: if you are going on routine in-state travel, you only have to fill out the form once per fiscal year. Routine In-State Travel is travel to a destination in Louisiana that occurs regularly during the course of the year. Only one original Authorization request per fiscal year is required for this type of travel.

    Here's an example:

    You travel to Baton Rouge once a month for a meeting.
