1. Print Application

2. Complete the Employee Information section (top), the Employee Acknowledgement Signature section (bottom) and the sign the Notice and Consent to the Processing Data
   form (2nd page). 
   
   The Approving Manager’s signature should be the Department
   Dean or Director.

3. Submit the approved application to the State Travel Liaison in Office of Financial Services.

The application process takes approximately 10 business days from date of submission to the Office of Financial Services.
